Sibling Survey

Dear family members,

Walnut Street Center is currently re-assessing its services to improve the supports the agency provides to adults who have developmental disabilities and their families. Walnut Street Center would like to hear from the siblings of the people we serve to find out how we can become a resource to them as well. The agency is aware that many siblings are becoming more responsible for the coordination of care for their brothers or sisters as their parents age. Please take the time to complete this survey, which will provide the agency with information about the appropriate resources to meet your needs.
