Individual Support
Walnut Street Center’s Individual Support Program empowers each individual to become more self-sufficient and engaged in the decisions that affect their lives. Services are designed to ensure flexibility and therefore meet the evolving needs of each individual. We meet you where you are and help you get to where you want to be.
This model provides case management based on personal need. Persons receiving this service live independently in community-based private homes. With the help of case management staff, they are able to receive assistance as needed while also obtaining the highest level of independence. Each individual is viewed holistically and we empower each person to achieve their personal goals and desires.
Individuals receive assistance with managing:
- appointments + treatments
- transportation
- entitlements + benefits
- additional support workers
- recreational activities
- financial management
- landlord relations + housing
